Dog Swollen Belly: Causes, Symptoms & When It's Serious

A swollen belly in a dog can have causes ranging from a too-large meal to a life-threatening emergency. Knowing the difference โ€” and knowing it quickly โ€” can be the most important thing you do for your dog.

Causes of a Swollen Belly in Dogs

Bloat and GDV (Gastric Dilatation-Volvulus)

GDV is the most dangerous cause of sudden abdominal distension in dogs. The stomach fills with gas and, in severe cases, twists on itself (volvulus), cutting off blood supply. This is fatal within hours without emergency surgery.

Signs of GDV: sudden, dramatic abdominal swelling; unproductive retching or dry heaving; drooling; restlessness or inability to get comfortable; pale gums; rapid breathing. This is a surgical emergency โ€” go directly to an emergency vet.

Deep-chested breeds are most at risk: Great Danes, German Shepherds, Standard Poodles, Weimaraners, Boxers, and Labrador Retrievers.

Ascites (Fluid Accumulation)

Ascites is the abnormal accumulation of fluid in the abdomen. Unlike bloat (which comes on suddenly), ascites develops gradually, giving the belly a progressively rounder, fluid-filled appearance. Causes include:

  • Heart failure โ€” the most common cause; fluid backs up into the abdomen as the heart fails to pump efficiently
  • Liver failure โ€” the liver loses its ability to produce proteins that keep fluid in the bloodstream
  • Cancer โ€” abdominal tumors or tumor rupture can cause fluid buildup
  • Kidney disease or protein-losing conditions

Cushing's Disease (Hyperadrenocorticism)

Cushing's disease โ€” excess cortisol production from the adrenal glands โ€” causes a characteristic pot-bellied appearance in affected dogs. The belly sags and enlarges due to muscle wasting and fat redistribution. Other signs include increased thirst, urination, appetite, symmetrical hair loss, and a dull coat.

Pregnancy or Pseudopregnancy

An intact female dog with an enlarged belly may simply be pregnant. A false pregnancy (pseudopregnancy) can also cause abdominal enlargement in spayed or unspayed females due to hormonal changes.

Pyometra

An infected uterus (pyometra) in intact female dogs causes significant abdominal swelling, lethargy, vaginal discharge, and increased thirst. This is a life-threatening emergency requiring surgery.

Intestinal Blockage

A foreign object (toy, sock, bone) that lodges in the intestines causes obstruction, gas buildup, and swelling. Signs include vomiting, not eating, abdominal pain, and straining without producing stool.

Obesity

Simple fat accumulation can give dogs a rounded abdomen, but this is uniform and gradual, without the rapid onset or additional symptoms associated with the serious causes above.

Emergency Warning Signs

Go to an emergency vet immediately if the swelling:

  • Came on suddenly (within hours)
  • Is accompanied by unproductive retching or dry heaving
  • Includes pale gums or collapse
  • Is in a deep-chested breed
  • Is accompanied by pain when you touch the belly

  • Note when the swelling started and what other symptoms are present
  • Do not give food, water, or medications until your vet assesses your dog
  • Intact female with swelling and lethargy โ€” assume pyometra until proven otherwise
